**Program Coordinator - Community & School Health Initiative**
**Northridge Hospital Medical Center**
Northridge Hospital Medical Center is seeking a passionate and dedicated **Program Coordinator** to join our **Community Health Work & Sustainable Outreach and Navigation (CHW SON)** team. This position is grant-funded and will play a vital role in the daily supervison of **Community Patient Navigators (CPN's)** , relays updates and orversees the implemtaion and refinement of the CHW SON project.
**Responsibilities**
+ Develop and implement program activities, including community health education workshops, outreach events, and resource referrals.
+ Manage program budget, track expenses, and ensure compliance with grant requirements.
+ Develop and maintain program materials, including curriculum, presentations, and promotional materials.
+ Coordinate with community partners and stakeholders to ensure program effectiveness and reach.
+ Identify and engage with community members, particularly those from underserved populations.
+ Build relationships with community leaders, organizations, and institutions.
+ Conduct needs assessments and community health surveys to inform program development.
+ Train, and supervise Community Patient Navigators (CPN's) to deliver program services.
+ Provide ongoing support and mentorship to (CPNs), ensuring their skills and knowledge are up-to-date.
+ Collect and analyze program data to track progress, measure impact, and inform program improvement.
+ Prepare regular reports for funders and stakeholders, highlighting program achievements and challenges.
+ Other duties as assigned
**Qualifications**
+ BS/BA degree required in Health Administration Public Health or Public Policy degree preferred.
+ Master's degree preferred.
+ 2 years of experience as a Community Health Worker or System Navigator or 5 years’ of experience as in community based programs or social services.
+ Minimum 1-2 years’ experience implementing community-based programs developing outreach strategies collaborating with community-based organizations and report writing.
+ Basic knowledge of budgeting or accounting.
+ Knowledge of local community agencies and resources.
+ Ability to organize projects and establish priorities.
+ Creative and analytical skills.
+ Ability to travel locally for outreach events (some evenings and weekends).
+ Grant writing experience preferred.
+ Presentation and public speaking skills.
+ Working knowledge of Microsoft 365 and Google Workspace products Zoom and Canva.
+ Bilingual (Spanish/English) preferred.
+ Fire Card (must obtain within 90 days of hire date. We offer the classes in the hospital)
**Pay Range**
$34.50 - $45.21 /hour
