Position Summary
The Case Manager - SMI coordinates treatment activity, participates in client staffing, implements treatment plans, provides rehabilitation services, provides general healthcare management activities, and maintains client records. As healthcare reform advances and health homes become the norm, the Case Manager - SMI is required to expand their capacity in healthcare navigation, build on existing skills in behavior health change, and gain a better understanding of the common health problems and basic interventions for individuals with mental illness and those with chronic health problems.
The Case Manager - SMI interacts with clients and staff in a culturally competent manner that takes into account factors such as age, developmental or acquired disabilities, religion and spiritual orientation, ethnicity, socioeconomic status, sexual orientation, indigenous heritage, national origin, gender, and gender identity. Enhances professional growth and keeps informed of evidence-based practices in behavioral health field related to populations served and modalities used to include working knowledge of DSM V, and its criteria for mental health disorders. Follows policies and procedures and performs other duties as assigned by supervisor.
The Case Manager - SMI assists with managing internal requests for providers and assists with coordination of high-needs cases among the department providers. Assists and coordinates with hospital discharges within the department and is responsible for providing a high level of crisis assessments and intervention in community settings.
The Case Manager - SMI is a full-time, non-exempt, hourly position typically working Monday through Friday business hours (some flexibility may be offered) and works on-site and in the community. Local travel will be required and company vehicles will be provided and/or mileage will be reimbursed.
Required Qualifications

High School Diploma or Associates degree and Minimum 2 years' experience in behavioral health; Bachelor's degree (any) and at least 1 year experience in behavioral health; OR Bachelor's degree in a human services related field.
Excellent customer service and client care.
Must support an access to care model.
Have or ability to obtain a Level I Fingerprint Clearance Card.
Maintain current AZ driver's license with acceptable driving record.
Maintain approved CPR/First Aid certification.
Successfully pass background check and drug screen.
Intermediate knowledge of DSM V diagnostic criteria and demonstrated ability to present clinical case presentation and signs/symptoms of a mental illness diagnosis.
Demonstrated ability to provide comprehensive and clinically appropriate crisis assessment and intervention activities.
Basic understanding of common health problems and interventions (i.e. obesity, smoking, nutrition, health promotion/wellness) and demonstration of skills in behavior health change.
Provides clinically appropriate care management activities including coordination of physical healthcare with Primary Care Physicians both internal and external.
Understanding of coordination of physical healthcare (fluency and demonstration).
Ability to navigate healthcare system.
Able to demonstrate skills in behavioral health change.
Excellent customer service skills.
Ability to interact with clients, personnel, and outside agencies calmly, logically, and respectfully utilizing cultural competency.
Ability to professionally document progress notes in a timely manner.
Ability to maintain confidentiality.
Ability to maintain professional boundaries.
Ability to effectively prioritize workload.
Microsoft Office/Internet proficiency and excellent computer skills.
Ability to use various types of office equipment (personal computer, photocopier, fax).
